Family health portrait

To help Americans gather family history information, the U.S. Surgeon General’s office has developed a free computer program that organizes health information into a printout individuals can take to their healthcare professional.

The printout can help the health care professional determine if a patient is at a higher risk for a disease.

The Web-based computer program, called “My Family Health Portrait,” is available at http://www.hhs.gov/familyhistory. To order a print version, call 1 888 878-3256 or write “My Family Health Portrait,” Pueblo, Col 81009.
